Patient Care Navigator (PCN)
At the heart of our services is the Patient Care Navigator (PCN) — your advocate, guide, and information. Every PCN has completed The George Washington University School of Medicine and Health Science's Oncology Patient Navigator Training: The Fundamentals, a nationally recognized program updated with CMS support. This ensures our team meets the Centers for Medicare & Medicaid Services (CMS) requirements for Principal Illness Navigation (PIN) services and follows the Professional Oncology Navigation Standards (PONT).
To guarantee consistency and excellence, our navigators follow the CARES™ Roadmap:
- C – Care Coordination: Aligning oncology, surgery, radiation, pathology, and supportive services so nothing falls through the cracks.
- A – Advocacy: Giving patients a voice, asking the tough questions, and ensuring your concerns are heard.
- R – Resources: Connecting you to financial support, home health, psych-oncology, nutrition, and community programs.
- E – Education: Breaking down medical jargon, explaining options clearly, and empowering informed decisions.
- S – Support: Offering compassion, encouragement, and steady guidance from diagnosis through survivorship or end-of-life care.
This roadmap ensures every patient is guided through a structured, consistent process — from first contact and onboarding, through active engagement, and ultimately case closure. At each step, CCC upholds its values of compassion, professionalism, and trust.

Cancer Care Concierge works directly with your physician and care team to provide navigation services that are recognized and reimbursable by Medicare under new CMS Principal Illness Navigation (PIN) codes. This collaborative approach ensures your medical care stays fully coordinated, while you benefit from the added support of trained navigators focused on your needs.

About Joseph A. Bauer, Ph.D.
Founder, Cancer Care Concierge
I started Cancer Care Concierge after my own family’s experience when my father was diagnosed with glioblastoma (GBM). Even with my background in cancer research, we struggled with confusion, gaps in communication, and the stress of not knowing what to expect. I saw firsthand how overwhelming the process can be for patients and families—and I knew there had to be a better way.
For more than 25 years, I have dedicated my career to cancer research, from developing new therapies at the Cleveland Clinic to leading biotechnology companies focused on cancer treatment. Along the way, I’ve worked with physicians, scientists, and patient advocates, published widely in cancer research, and helped move discoveries closer to the clinic.
I am certified in Oncology Patient Navigation from The George Washington University School of Medicine and Health Sciences which meets CMS Principal Illness Navigation (PIN) and
Professional Oncology Navigation Standards (PONT).
But what matters most to me is helping families like mine. Cancer Care Concierge was created to make the journey easier—to answer questions, connect patients with resources, and help families feel supported every step of the way.
My mission is simple: to take away as much of the fear and uncertainty as possible, so patients and caregivers can focus on what truly matters—healing, hope, and time together.
